Step 1. Complete your paperwork
Please click on the link below to start downloading your new patient paperwork. You can also find the same paperwork on the [Home] Page of this website. Please fill out all required fields within the secured paperwork. Upon entering a signature, you'll be asked to set a Digital Signature. This Digital Signature ensures personal authenticity and security.
Step 2. Activate your Secure Messaging Portal
In just a few short steps, you'll need to register your completely secure self care portal in which you will be able to send private messages to your physician. Everything is encrypted for your protection and confidentiality. If you already have a secure messaging profile, you can move to Step 3.
Step 3. Submit your paperwork
Once you've completed your paperwork, saved a copy for your records, and registered your secure portal, you're ready to submit straight to the office!
